By creating an account or using Quarterback Health ("QBH," "the Service"), you agree to these Terms of Service. If you do not agree, do not use the Service. You must be at least 18 years old to use Quarterback Health.
Quarterback Health is a healthcare coordination platform that helps you organize your providers, manage appointments, and handle health administration tasks. Our AI assistant, Kate, can chat with you about your care, call provider offices on your behalf, and suggest next steps for your health management.
Quarterback Health is not a healthcare provider and does not provide medical advice, diagnosis, or treatment. Kate is a care coordination assistant, not a medical professional. Any suggestions, summaries, or recommendations provided through the Service are for organizational and administrative purposes only. Always consult qualified healthcare providers for medical decisions. Do not disregard or delay seeking professional medical advice based on anything provided through this Service.
By authorizing AI phone calls, you understand and agree that:
You are responsible for maintaining the security of your account credentials and for all activity under your account. You agree to provide accurate, current information during registration and to update it as needed. You may delete your account at any time from the Account page, which will permanently remove all your data.
You may connect third-party services (bank accounts via Plaid, Google Calendar, Outlook Calendar) to enhance your experience. When you connect these services, you authorize us to access the specific data described during the connection process. You can disconnect these services at any time. Your use of third-party services is also subject to their respective terms and privacy policies.

If you add care recipients (family members or others you manage care for), you represent that you have their consent to manage their healthcare information through this Service, or that you are their legal guardian or authorized representative.
When you upload health documents, they are processed by AI to generate summaries and are stored in encrypted storage. You retain ownership of all documents you upload. Summaries generated by AI may not be perfectly accurate — always verify important information against original documents.
The Service, including its design, features, and content (excluding your personal data), is owned by Quarterback Health and protected by intellectual property laws. You are granted a limited, non-exclusive, non-transferable license to use the Service for personal, non-commercial healthcare coordination purposes.
To the maximum extent permitted by law, Quarterback Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Service. This includes, without limitation, damages arising from missed appointments, incorrect AI summaries, failed phone calls, or reliance on any information provided through the Service. Our total liability shall not exceed the amount you paid for the Service in the twelve months preceding the claim.
The Service is provided "as is" and "as available" without warranties of any kind, either express or implied. We do not warrant that the Service will be uninterrupted, error-free, or that AI-generated content will be accurate or complete. We do not guarantee that any appointment will be successfully booked or that provider information is current.
We may suspend or terminate your access to the Service at any time for violation of these Terms or for any other reason with reasonable notice. You may terminate your account at any time by using the "Delete my account" feature. Upon termination, your data will be permanently deleted.
We may modify these Terms at any time. We will notify you of material changes by email or through the Service. Continued use after changes constitutes acceptance. If you disagree with updated Terms, you should stop using the Service and delete your account.
These Terms are governed by the laws of the State of Connecticut, without regard to conflict of law principles. Any disputes shall be resolved in the courts located in Fairfield County, Connecticut.
